Please ensure that the Student Request Form is filled out completely, accurately, and in a timely manner to avoid any delays in processing. Incomplete or incorrect information may result in scheduling issues or the need for re-submission.

Please answer the following questions at least three working days before the exam is to be taken.

Remember to bring a current and original ID such as a 51AV��Ƶ Student ID card, driver's license, passport, military card, or permanent resident card. An expired ID and a picture of your ID will not be accepted.

Plan to arrive 10 to 15 minutes before your appointment to allow time to check-in.

Please let us know when you find out you will be late or miss your appointment. That way, you are not marked as a No-Show.
